    Birth date from April 2, 2011 to April 1, 2012

Examination Dates

 

Observation in a group-play setting Nov. 4, 2017 (Sat) or Nov. 5, 2017 (Sun) in the morning

Child and parent(s) interview

Date: Sept. 25, 2017 (Mon) – Oct. 24, 2017 (Tue) *except Sat., Sun. and holidays.
Time: 10:00 - 15:30 (approximately 15 minutes per child)
*Advance Reservation is required.

Required Documents

 

1. Application Form (Designated Form)
Please attach a color photograph (with the child’s name on the back) taken within the last 3 months.
2. Proof of Payment (Receipt) of Application Fee
Please attach the proof of payment (receipt) after the bank transfer to page 3 of the application form.
3. Copy of an ID to Verify Applicant’s Date of Birth
Please attach a copy of the applicant’s ID where date of birth is shown to page 3 of the application form.
4. Examination Slip (Designated Slip)
Please attach a color photograph to the examination slip and fill in the required fields.
5. Letter of Agreement
Please read through and fill in the required fields.
6. Health Certificate (Designated Slip)
Please get a health examination and have a doctor fill out the required form.
7. A Set of Two Label Stickers for Mailing Address (Designated Stickers)
The labels will be used to send “Examination Slip” and “Entrance Examination Result”. Please write the exact mailing address. If destinations are different addresses, please write “Examination Slip” or “Entrance Examination Result” on the bottom right of the stickers.

Payment Deadline
Payment must be made between Sep. 19, 2017 (Tue) to Sep. 29, 2017 (Fri). You will receive a proof of payment (receipt) after the bank transfer. Please attach it to the application form.

Submitting Documents and Deadline

 

How to Submit Documents
You can send the required documents by mail or submit in person to the administration office. Please use the registered mail or courier when mailing the documents to prevent loss. The administration office is open to accept the application from 10:30 to 17:00 (Except on the first day, from 10:00) on weekdays. Any application sent by fax or e-mail is NOT ACCEPTED.
Please make an appointment for the child and parent(s) interview by referring to “the Child and Parent(s) Interview Information” in the separate sheet.
