Requests for application materials to be posted by mail will be accepted from the Google Form below until 16:30 Tuesday, January 6. 

Application forms will be sent out in order from January 5.

We will only send application forms to families in Japan. If you are living overseas and wish to receive an application form, we will send you by email.
※We cannot ship to US military base addresses in Japan. Please pick up Application forms at the administrative counter.

**Please note that the application documents include a medical certificate to be completed at a medical institution and a certificate of enrollment from your current school (or public school in the case you enrolled an international school in Japan). Please make your appointments and arrangements ahead of time.
